Determining eligibility
To determine eligibility for emergency assistance, the following documents are required:
- Social Security cards for all household members.
- Income documentation for at least the past three months for all household members who receive income or assistance.
- Proof of residence showing a current Volusia County address (we cannot accept an address with only a post office box number).
- A picture identification card, preferably a Florida driver's license.
- Proof of a crisis in the home which has caused an interruption in the total family income, i.e., unemployment, disability, desertion, etc.
- Proof of monthly expenses for the month (receipt of expenditures).
In addition the applicant must present the following documents:
- Rental assistance - three day eviction notice.
- Mortgage assistance - foreclosure notice or mortgage payment book.
- Electric assistance - past due electric notice.
- Prescription assistance - prescription from physician.
Assistance cannot be provided without these documents.
