PETITION FOR SPECIAL ASSESSMENT DISTRICT

To Volusia County Council:

WHEREAS, the Volusia County Council, pursuant to Ordinance No. 79-2, may establish Special Assessment District; or pursuant to Ordinance No. 79-3 may establish Special Service Districts.

The undersigned petitioners, being owners of property situated within the area hereinafter described, do hereby petition the Volusia County Council for the following:

Section I. Purpose. That a Special District be formed for the purpose of providing ____________________ (Streetlights, water, sewer, streets, etc.)

Section II. Boundaries. That the area to be included in this District shall embrace and include the following described as:
